Job Description
Full-time LPN Position $30-$40/hr DOE: POSITION SUMMARY The Assistant Health Services Director assists the Health Services Director in ensuring resident care needs are met by health and supportive services provided to residents as directed by health services leadership. Assists health services leadership in overseeing health services procedures and systems, care staff, and schedules. ESSENTIAL DUTIES AND RESPONSIBILITIES 1. Partners with health services leadership to create a resident-centered approach to the planning and delivery of services to residents, supporting residents' rights to participate in decision-making and treating residents with dignity and respect. 2. Assists with assessments or evaluations on health, functional and psychosocial status of residents. Participates in the development of individualized service plans. Proactively manages care and services for each resident, maximizing the resident's opportunity to remain in the assisted living environment. Problem-solves as part of the management team to case manage and meet the needs of residents. 3. Evaluates residents with reported changes of condition. Provides team member direction, service planning, and coordination of care to meet the residents changing needs. 4. Collaborates with the management team on the move in of prospective residents, taking into consideration the needs of the prospective resident, current community acuity, and staffing levels. 5. Assists in the coordination of resident health services appointments for offsite visits through transportation services or use of the company vehicle. 6. Communicates regularly with residents' families regarding health care needs. Provides education and support to residents and families. 7. Assists in supervision and training of health services team members including personal care and medication management. 8. Partners in the investigation and root cause analysis of incidents. Collaborates with the management team on the development and implementation of appropriate interventions. 9. Collaborates with Health Services Director to ensure the documentation and communication of resident care and services. Ensures resident records are completed in a timely manner and maintained according to company policies and state regulations. 10. Assists in the management of Infection Control. Provides training to staff on infection control practices, investigates, and reports incidents of infection and tracks infection data. 11. Provides on-call support and phone triage for team members' questions regarding resident concerns. Work as a Medication Aide and/or Personal Care Attendant as needed. 12. Performs other duties as assigned. COMPANY WIDE RESPONSIBILITIES 1. Demonstrates Core Values of Respect, Integrity, Commitment, and Trust. 2. Maintains a safe and secure working environment and practices safe working habits. QUALIFICATIONS AND EXPERIENCE Must have a current, unencumbered LPN/LVN license. Excellent written and oral communication, computer and motivational skills are essential to success. Ability to creatively problem-solve in both resident care and employee management situations. Experience in geriatric nursing, medication administration, case management, training/staff development, budgets, and team management a plus. Additional course work in management an asset.
