SUMMARY:

The LEAD Screening and Intake Coordinator works as a member of a multi-disciplinary team providing screening, outreach, and service brokerage to participants referred to the LEAD program. The team coordinates with community resources to broker services provide access to address client needs, including substance abuse treatment, medication management, physical health treatment, mental health treatment, benefits and entitlements, appropriate and affordable housing, employment and educational opportunities, and supports for basic needs.

The LEAD Screening/Intake Coordinator will act as the contact point for all referrals to LEAD case management. As such, this position must ensure effective and efficient communication and collaboration between all the partners involved in referring and receiving referrals for LEAD. This position will provide screening, outreach and engagement, and short-term care coordination to individuals referred to LEAD while awaiting engagement from the assigned long term LEAD CM.

RESPONSIBILITIES:

  • Provide street outreach to engage individuals who have been referred to LEAD with the purpose of facilitating completion of intake process.
  • Develop and maintain positive, collaborative relationships with all LEAD partners (such as Law Enforcement, Human Services, District Attorney's office) and other service providers in order to best serve LEAD participants and serve as direct liaison regarding specific referrals for LEAD engagement and participation in this service.
  • Complete initial screening and intake with all new referrals.
  • At time of screening, assess and support accessing services for emergent/immediate client needs including need for medical care, shelter, food, and clothing.
  • Provide street outreach and short-term case management services to engage individuals who have been referred to LEAD with the purpose of facilitating completion of intake process. The Intake coordinator will provide bridge/transitional CM supports until the regular assigned CM has capacity to engage the participant in longer term case management services
  • Manage, coordinate and facilitate the assignment/hand off of new referrals to a case manager, peer or housing.
  • Provide the assigned case managers with completed screening paperwork, a warm hand off from any short term case management provided and share additional, relevant information. Help to facilitate successful warm hand off to long term LEAD Case Manager.
  • Work in tandem with case managers to provide street outreach to engage participants when needed.
  • Assist Program Supervisor to maintain documents listing all LEAD participants. Update demographic information, referral date, and type of referral, arrest charge and case manager assignment.
  • Attend all LEAD Program staff meetings and LEAD Operations Team meetings.
  • Maintain all records as required.
  • Work within scope of training and experience and seeks consultation when appropriate.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  • Attend all mandatory CCC trainings in a timely manner.

DESCRIPTION OF OTHER DUTIES:

  • Participate in Outreach activities necessary to offer services and engage participation in a non-coercive manner. Escort individuals to meetings and appointments as necessary;
  • Participate in admission and discharge planning for any client needing hospitalization or subacute level of care.
  • Provide testimony, advocacy, and care coordination with courts and other agencies
  • Collaborate with police, sheriff's department, courts and other agencies
  • Participate in training opportunities to advance skills and knowledge in addictions and mental health.
  • Perform other duties as assigned.

ABILITIES:

  • Basic knowledge of Motivational Interviewing, Stages of Change, ASAM and Harm Reduction.
  • Demonstrated knowledge of community and social service agencies, including health services (substance use, mental health, and physical health), housing, employment, benefits, and other social services.
  • Computer literate, with basic knowledge of Microsoft Office Suite, as well as a high level of initiative in keeping current with technological change.
  • Ability to prioritize workload and activities of self and complete tasks in a timely and efficient manner.
  • Ability to set boundaries, resolve interpersonal conflict and de-escalate issues.
  • Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving.
  • Ability to establish and maintain effective working relationships with staff, clients, and outside contacts from a wide variety of ethnic, socioeconomic and cultural backgrounds; good diplomatic skills.

  • Must be able to climb stairs several times a day.

QUALIFICATIONS:

  • HSD/GED required.
  • Education and experience appropriate for this role, as demonstrated as one of the following: QMHA-R, CADC-R, Bachelor's Degree in service related field preferred. (This is not a privileged position.)
  • Experience working with homeless individuals and providing street outreach, or outreach based community social service or healthcare environment preferred.
  • An undergraduate degree in a behavioral health field preferred.
  • Lived experience preferred.
  • Bi-lingual preferred.
  • Must have current CPR certification prior to start date.
  • Has the competencies necessary to:
    • Communicate effectively
    • Perform drug and alcohol assessments based on ASAM criteria, understand treatment and service terminology and able to apply the concepts
    • Assist participants in developing a self-directed plan.
  • Ability to adhere to Central City Concern's drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Ace of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol.
  • Must pass a pre-employment drug screen, TB test, and background check. This includes clearance by the DHS Background Check Unit.
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
  • Must possess a current driver's license, pass a DMV background check and be designated an "acceptable" driver as outlined in Central City Concern's Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
  • Must adhere to agency's non-discrimination policies.
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.
